Supported Living

Our Supported Living Services enable people to live in their own home and receive care and support from Cedar Health and Wellbeing, which is regulated by CQC.

Supported Living is a way to support individuals to live as independently as possible. The Supported Living model enables individuals to live in their own homes, make their own choices and enjoy the same rights and responsibilities as anyone else, while receiving the level of support that’s right for them.

At Cedar Health and Wellbeing, we provide Supported Living specifically for adults with Autism and/or Learning Disabilities. Our approach is tailored to meet each person’s unique needs, strengths and aspirations, helping them to live safely, confidently and independently within their communities.

The accommodation is either the individual’s own home by ownership or by way of a tenancy with a specialist registered housing provider. Each person has their own tenancy, provided in an easy-read document and that is in line with NDTI real tenancy test, where we support people to manage their tenancy.

We uphold the Reach Standards, a collection of nine voluntary standards developed by Paradigm, to guarantee that people are supported in leading the lives they choose, while enjoying the same freedoms, rights and obligations as everyone else.

Support is provided 24 hours a day and is tailored to their individual needs i.e., daily living, personal care, medication, meal preparation and housekeeping. The goal is to give each person the ability to be as independent and autonomous as possible.

We recognise that every individual has their own unique goals and aspirations. Our experienced team works closely with each person, their families and all professionals involved, to develop a personalised support plan tailored to their specific requirements and goals.

Supported Living means having your own home or tenancy, with tailored 24-hour support to help with things like daily living skills, personal care, medication, activities and household tasks. The goal is to support each person to live as independently as possible, with plans tailored to their needs, choices, and aspirations.

Our Supported Living Services are suitable for adults with Autism and/or Learning Disabilities who need support to live independently. We work closely with each individual to create a personalised approach that reflects their unique needs and goals.

In Supported Living, people live in accommodation that is either owned by them or rented through a tenancy agreement. Supported by staff, they will be responsible for their bills and every day living costs, giving them greater independence and choice. In our Residential Services, all costs are funded and individuals do not have this responsibility.

Our support is designed around the individual needs and wishes of each person. This can include help with daily living skills, personal care, medication, meal preparation, housekeeping and taking part in activities. Our excellent staff teams have all received comprehensive in-house training and are available 24/7 to ensure each individual feels safe and supported.

Outreach Support

Our Outreach Support involves assisting people living in their own home, often with family. We work alongside families to support a person with all elements of their life, including personal care and accessing community activities.

How much support we provide is determined by working in partnership with each person and their circle of support, to develop their support plan.

The care and support we provide is also regulated by the Care Quality Commission.
